Tesamorelin vs Ipamorelin

Both act on the pituitary and neither supplies growth hormone. They reach the same gland through different receptors, and that is the distinction the category is built on.

Tesamorelin is a GHRH analogue: 44 amino acids, stabilised against enzymatic degradation, binding the GHRH receptor — the same route the hypothalamus uses. Its human data sits almost entirely in one indication, HIV-associated lipodystrophy, measured on visceral adipose tissue. Broad conclusions about body composition generally do not follow from a single indication, and the literature should be read with that boundary visible.

Ipamorelin approaches from the other side. It is a pentapeptide agonist at the ghrelin receptor, GHSR-1a, selected specifically for releasing growth hormone with comparatively little movement in cortisol and prolactin. Where hexarelin and the older GHRPs carry off-target endocrine activity, ipamorelin was chosen not to.

These are the two mechanistic classes, and the reason the catalogue also carries a blend of one from each. Choosing between them singly is choosing which receptor the question is about — not which is stronger.
