HGH is recombinant human growth hormone — a 191-amino-acid polypeptide identical to the pituitary hormone, acting directly at the growth-hormone receptor and downstream on IGF-1. Supplying it bypasses the pituitary entirely, which means the pattern of exposure is whatever the administration schedule makes it, not what the gland would have produced.

Tesamorelin is a stabilised GHRH analogue of 44 amino acids, modified with a trans-3-hexenoic acid group to resist enzymatic degradation. It acts a step upstream, at the pituitary, so any release stays subject to the feedback and pulsatility of the axis itself. Its human evidence is unusually specific: nearly all of it comes from one indication, HIV-associated lipodystrophy, with visceral adipose tissue as the measured endpoint. That is a narrow base to generalise from, and it should be read as narrow.

So the choice is between direct receptor agonism and upstream stimulation with the gland still in the loop.
