What you are actually choosing between

Both are ghrelin-receptor agonists. What separates them is selectivity, and what that selectivity is traded for.

Hexarelin is a synthetic hexapeptide and the more promiscuous of the two. Beyond GHSR-1a it carries documented affinity for cardiac CD36, which is why a meaningful part of its literature is cardiovascular rather than endocrine — a body of work with no counterpart for ipamorelin. The same breadth shows up in the endocrine data: hexarelin's published profile includes movement in cortisol and prolactin, and desensitisation on repeated administration is a recurring theme in the studies rather than a footnote to them.

Ipamorelin was selected for the opposite property. It is a pentapeptide picked out in screening for growth-hormone release with comparatively little disturbance of cortisol and prolactin, and that narrowness is the entire point of the molecule.

So the decision is not which is stronger. It is whether the research question wants a broad ghrelin-receptor probe, with the off-target activity and tachyphylaxis attached to it, or a clean single-axis one. Those are two different experiments.
